A lot of people think of muffler flow as a straight line or stream. It's not. The modern chambered mufflers uses a plenum at either end and relies on the pressures of the exhaust pulses to flow and scavenge while the baffles generate sound cancelling waves back towards the incoming sound waves.

The location of the outlet relative to the inlet is a matter of the best position for the pipes for ground clearance and clearance to shocks and rear axle. in most cases, the maker locates the pipes where the OEM hanger mounts are. On an A Body, that is usually offset inlet and center outlet.
