Jo-Ann fabric, Michael's - any sewing/crafts type store will have the foam. You can also shop on ebay as well.
When I do mine, I'll try ebay first (online is usually a little cheaper than retail)
Also, you can usually find a decent electric knife at goodwill/thrift stores cheap- for this sort of project.
