Default Readable PHS form headers

My '74 PHS which I've had for years has readable vehicle information, but the column headers and info on the pre-printed form are impossible to read. Has anybody ever gotten one clean enough to read? I realize the info on the invoice changed from year to year.

Here is a rough copy of 71 (and I assume 72) that I cleaned up so it is readable.
If I made any mistakes, let me know.
The text at the bottom changed for 73-74, but I haven't found a clean copy yet.

Are you sure of the ADV ASSN COST AMT?

On a '73 it changed to ADV ASSN. COLL. AMT
FDH changed to D & H and only listed once above.
Lower had D & H AMOUNT *
Lower right had box for TOTAL INVOICE PRICE, still had
LESS HOLDBACK
LESS ADV. ASSN. COLL. and
TOTAL INV. LESS H/B & ADV. but no boxes.
* D &H AMOUNT REFLECTS PROVISION FOR PASS THROUGH OF TIRE WEIGHT TAX IMPOSED ON MANUFACTURER OR IMPORTER OF TIRES. SHIPPED TO - SAME (UNLESS OTHERWISE INDICATED)
Above lower line (right hand) P4202 (O.D.C.) REV 12-70

'74 - DATE OF DRAFT changed to DATE OF NOTE
and all the extra stuff at the bottom was added.

Thanks. The copy I found is not perfectly clear, so I have assumed some things. Computer is out for repairs, so I'll get back to it next week.
Being printed in the 70's, the fonts are different and letter spacing is very inconsistent.
