Karl,

To answer your questions about the camshaft & followers, I have never seen damage to a cam follower like you have on the #4 Exhaust follower, it seems to have damaged the lobe some by your picture. Also it looks like most if not all the followers have visible wear on them. Usually when the follower is worn that much the cam lobes are also worn down a like amount.

Another thing to check is the Oil hole from the block to the head, in 1966 and some early production 67 engines they had an issue with the alignment of the pressure oil hole from the block to the head. I have seen it misaligned so less than 1/2 the oil passage to the head matches up to the hole in the bock. It is easy to correct if it is there, I just hand ground the hole so match up, and modified the head gasket to match. This issue caused alot of early cam failures. It was corrected early in the 1967 model production. So not an issue with later engines.
