I finally found the box I was looking for and the one I have is a 9097, it is in excellent condition if that helps you out. According to the box and paperwork inside, I purchased this from Jim Hand years ago for our 69 Firebird.

Interesting! I'm not sure what to make of it. I have been told that some of the original RA IV cars came with ones with another number on them that is crossed out like this one and re-stamped with 9099 by the factory. I have not seen or heard of one with 9099 crossed out. PM sent.

I just noticed this!

That picture I posted - probably because of the lighting, made me think I was seeing a "0" and an "8" to the left of the number - obscured by rust;
I just lightly wire wheeled it, and sure enough - to the left of the lined out 9099 is a 1094

EDIT:

So based on Scott's handy list, was this maybe a left over 9099 that was renumbered to use on a 1971 455HO?

Yes, the 1094 is the service replacement for many applications including the 9099. I have an NOS 1094.
