Default Pic Please

Can someone post a picture of their GM VIN door decal from a Norwood built car? Mine was missing so I want to be sure of what it is supposed to look like with GVWR and dates before ordering mine. If you are worried about revealing a VIN number, cover it up as I don't need that info.

Info sheet shows a dash for the month-year, but most I've seen are a slash on the month/year.
