How do you use the codes? Online or over the phone?

Should be able to use them either way. I always order online and enter the code in the "gift card or summit bucks" box and the date (in this case 07/09/2014) in the "issue/expiration" box.

Tried all 4 numbers and got this error message, "The gift card number (or coupon) you've supplied is either invalid, has an incorrect expiration date, or has not been shipped. Please check the numbers and expiration date you entered to be sure they were typed in correctly." I triple checked the expiration date.
