So what does the adjustment do? Change ride height (without any modification to springs)? And if yes, that can adjustment be done while installed in the car?

No it does not change the ride height.
You compress the shock all the way and you'll get it area where it locks in and then you turn it and it makes it stiffer and stiffer as you turn it.

These shocks are excellent for tuning your suspension for a level launch. We used them on the car in my avatar. By staggering the extension/compression adjustments, you can tailor how the four corners of your car react to the torque applied to the chassis when a vehicle hooks up and launches. They control the twist normally seen with stock suspension.
You do have to remove them to compress the piston to make changes; however, I fabricated a tool to do them on the car. :)
