There was a supplier problem in early 69, therefore Judge glove box emblems weren't put on cars until sometime in early february, 1969.
To answer your first question, it is actually possible to have a Judge built prior to mid February that has both the nameplate ensignia and the emblem.
I would say a build date prior to 2/10/69 would not have an emblem on it.
Conversely, I doubt any Judge built after around 2/20/69 had the GTO nameplate over the heater controls. However, I don't believe any A/C cars had the dash nameplate.
