Here's a Canadian Question:

On the top of the PHS it says:

"Charge to: GENERAL MOTORS of CANADA, LTD, OSHAWA, ONTARIO"

Is this the normal way a Canadian PHS looks? Or should the original dealer be in that spot on the form?
