will be interesting to see if they find it, note that they want you to sign/initial in the page two blank that applies and send a copy of your Texas DL in with the form. Who did you have your auto insurance thru if you remember? If they are still in business you never know what they might have kept as it would have had the VIN on it. Long shot but just thinking of things that might have the VIN on it
