I believe on my 67 I put those into the Wheelwell with the flat part (the part you screw into) facing the tire.

Then that wheelwell lip goes ABOVE up inside the fender lip. so the order is (from the ground up)
Screw, Fender, Plastic insert - Flat side, Wheelwell

Make sense? i don't know if that is what you are unsure about.
