It's why degreeing these things is important these days. I haven't had one yet that was dot to dot and came out to where the cam card wanted it to be, let alone where I wanted it to be. They have always been off a few degrees, some worse than others. It's why I like the 9 keyway sets so I can move things around and find one that works. The 3 keyways just don't give enough options.
It's very likely that it was where it was because they degreed the cam and put it on an ICL they wanted. It's why I mentioned before that pulling the cover you may not find anything wrong, since finding that the dots may not line up exactly isn't going to tell the whole story without checking things with a degree wheel.
You also have to make sure that when you line up the dots at either 12 and 6 or 12 and 12 that the #1 piston is on the compression stroke before you drop the distributor back in and phase it with #1 wire.
