Default Identify Shift Plate

Good morning,

I would appreciate any help in identifying the shift plate from my car. I've attached a picture. It has the following numbers on the back;

64095 & 9776299

64079 & 9776298

65136 9781520

I'm the 3rd owner so I don't know for sure but I'm assuming the car originally came with a 2 speed powerglide and one of the previous owners replaced it with a TH350 (and a 400). I want to buy a new 3 speed shift indicator, backing plate and lamp housing for a TH350 but don't know if it will fit this shift plate. Any help would be appreciated. You all have a good day.
