The 1963 "IDENT-O-PLATE" was metal and in two parts:



and was glued to the inside back cover of the Owner Protection Plan booklet:



(Not the original booklet or IDENT-O-PLATE to my car. The stub of the last page of the booklet contained three perforated "verification vouchers" to be signed by the owner upon request for warranty service, attesting to the fact that the factory-specified maintenance had been undertaken, and that the dealer had explained the scope of the warranty services provided - I still have two left in this booklet.)

I'm not sure when the name changed to the PROTECT-O-PLATE, but I do know that in my '68 Deville, I have the "original" PROTECT-O-PLATE (in plastic, the size of a credit card, as previously mentioned), embossed for use with a Bartizan manual credit card slider, with my dad's name on it. Now, my dad was the second owner of that car, which he bought for my mom in 1970. The reason for the PROTECT-O-PLATE having his name on it is that it was purchased as a "certified pre-owned" car and came with a warranty (whether dealer-only or factory-backed I could not say). I don't have that car near me at the moment, but the PROTECT-O-PLATE is still in the glovebox.
