Anyone know what a # 15 head belongs to? Obviously not a 1968 GTO.......

Look for the date codes.

If it's a 1968 head,it came on a 400 for the big cars.

They made a 15 head in 1970 for the 455 engine,also for a big car usually.

What date code and where will I find it?

I used your link to verify 1970 455, but not sure where to find the date......
