Default 68 mt clutch fork

I have been using a Lakewood bellhousing, clutch fork and adj. pivot ball with a Hayes 85-103 clutch kit for years. Putting car back to original bell housing using the hayes kit. Can anyone tell me the original GM part number for a 68 A body clutch fork? thanks in advance Mike

Thanks, I bought one off of ebay that I suspected was wrong. Supposed to be part no. GM 5014090. The guy advertized it to fit too many different models and it looks almost identical to the lakewood LW15500. You just saved me a helluva lot of work. Mike
