My 67 GTO has an 09D (4th week Sep) date code. Would this be an early (66) or late (67) build?

I too would say early build in 66 for your 67.
From my days of hauling new cars out of the GM plants in the late 70s, thru to Nov 1993, they shut down production in very late July at the earliest or anytime in August for the change over for the next year model. If no major changes were made, like in the change from 66 to 67 for the GTO, they would start production in about 30 days minimum or more depending on what was changed. Your 4th week would be about right for back then due to the front and rear changes. Not major changes but they were significant enough for a late September production line start up. I think Keith Seymore would agree on that. As side note. The complete model change like going from rear wheel drive to front wheel drive, or a completely different car, would take at least a year at the minimum. We went thru that 3 times when I hauled out of the GM plant in St., Therese, PQ Canada.

Gunnell's "Standard Catalog of Pontiac" shows 1967 production started 8-29-66 and finished 7-30-67.
So I would also say early. You can confirm by looking at some of the casting codes through out the car. K
