Default Buffalo Nickel

I did some grocery shopping today at Whole Foods and gave out some paper and received back some coins.

I didn’t pay attention at the store, but when I emptied my pockets at home, I was shocked!
In the change I got back was this 1928 Buffalo Nickel in with shiny pennies and dimes.
How did this happen in Oct. 2018?
It’s pretty worn and has a gash in the Indian forehead.

90 year old coin and still in circulation.
