Sounds like you have something broken or flexing bad in the linkage. If the t-o bearing isn't moving and you can press the pedal to the floor, something is broken, have a helper work the clutch pedal and put your eyes on everything. If the pressure plate was frozen you should not be able to push the pedal.

On the clutch fork, what is the relationship of the push rod pad to the centerline of the driveline?
Is the fork 'ribs' making contact with the bell housing when at rest? That would be an improperly adjusted ball. Take a pic and post. .
