4 speed code on PHS for 69 TA

Does anybody have a copy of the 69 order sheet that comes with the PHS packet? I can not read mine. I need to know what the 356 code is for the 4 speed trans.

M20 should have 2 grooves on the input shaft, M21 will have 1 groove on input shaft.

Thanks for the pictures they are great. Do you have pictures of the inside (from the bottom) of the housing where the lever contacts the spring? Mine fell apart and I am not sure how it is assembled.

What are the correct color stripes painted on the diveshaft for a 4 speed?
I can make out what appears to be a medium blue band about 12 inches from the front of the tube.
